Transition Skills Associate

Salary: $13.45 per hour


  • Implement curriculums that will include but not be limited to work preparedness, social skills, and life skills.
  • Act as case manager for assigned individuals.
  • Provide direct supervision and support to all individuals participating in the social, recreational, therapeutic, or educational programs within the local community.
  • Design and implement individualized social/recreational activities both in-house and in the community for individuals to successfully participate.
  • Collect and maintain data related to consumer goals.
  • Coordinate and implement vocational/socialization/educational programs.
  • Act as primary facilitator for Plan of Service meetings for development/implementation of service plan.
  • Develop placement/career plans, evaluation/intern sites and community based jobs.
  • Plan, design, and implement group activities based on DDS standards and individuals’ employment goals.
  • Provide direct supervision to individuals in a work setting which may include, but is not limited to: teaching new work skills, developing natural supports, reinforcing good work habits, establishing rapport with onsite employer, compliance with volunteer/training site rules/regulations.
  • Collect data and complete all paperwork in compliance with all DDS, CARF and agency standards/requirements.
  • Develop and facilitate work readiness curriculums, job shadowing, volunteering and industry-specific learning opportunities.

  • Bachelor’s Degree in Psychology, Social Work or related field. Associate’s Degree will be considered with two years of experience in the field.
  • One year experience in working with persons with disabilities preferred.
  • Experience working with culturally diverse populations.
  • Group facilitation experience preferred.
  • Valid Drivers License.
  • You are required to be in possession of a car on site during your entire shift to transport yourself and/or consumers to community sites in all weather conditions and throughout all neighborhoods.
  • Depending upon assignment, must be able to work in a variety of settings, weather conditions and locations.
  • Must have physical endurance and skills to participate in community recreation/social programs/classes for up to 90 minutes.
  • Must maintain CPI, CPR and First Aid certification.
  • Knowledge and skills in Word, Excel and the internet.
